    Case Study Analysis

    Scenario

A 42-year-old man comes to the clinic with a chief complaint of pain, redness, and swelling of his right calf. He states that he had been working in his yard using a string trimmer when the trimmer slipped and cut his leg. He cleaned the wound with water from the garden hose and covered the wound with a large Band-Aid. Several days later, he developed a fever of 100.6? F and chills and noticed that his leg was swollen and red. He comes to the emergency department for definitive care.

Purpose statement: The purpose of this essay is to evaluate and analyze the presented case scenario.

    Introduction

Following an injury, wound healing is a critical phase in tissue regeneration. While most wounds are easy to heal, some often result in adverse consequences such as inflammation, redness, and increased severity. Surface injuries often provide entry for pathogens to attack delicate body cells; thus, close interaction with medical personnel is necessary. This essay analyzes a presented case to explain the symptomology, genetic connections, immunosuppression, and the effect of the patient clinical status on body systems.

Explain why you think the patient presented the symptoms described.

The patient cut himself with a trimmer, which could have harbored microorganisms.  Besides, the patient used garden water without a disinfectant, further predisposing his wound for infection by waterborne pathogens. These microorganisms could be the cause of infection of the wound leading to the presented symptoms.  Inflammation is often a body reaction to tissue or cellular injury.

Typically, inflammation is symptomized with pain, redness, and swelling in the affected area (McCance & Huether, 2019) as presented by the patient. The patient’s pain is a result of swelling caused by pressure from exudate buildup and biochemical mediators for wound healing such as bradykinin or prostaglandins.

The patient’s fever results from cytokines – endogenous pyrogens, which interact with the body’s nervous and endocrine systems to modify defense against infection, causing the production of prostaglandins – the fundamental mediators of the regulated responses that contribute to fever. The patient’s symptoms could have been escalated by wound conditions that encouraged bacterial growth rather than host protection.

Identify the genes that may be associated with the development of the disease.

The patient’s symptoms indicate a bacterial infection often characterized by swelling and redness of the lower legs.  While host factors influence infection susceptibility, no studies are specifying particular genes influencing bacterial infection. Despite the limited studies on genes associated with bacterial infections, a genome-wide study conducted by Rogne et al. (2020) identified Solute Carrier Family 12 Member 2 to be associated with skin and soft tissue infections.  However, specific factors such as tobacco use, overweight, and obesity exacerbate incidences of skin and soft tissue infections (Rogne et al., 2020).

Explain the process of immunosuppression and the effect it has on body systems.

Immunosuppression refers to a condition in which the body’s immune system functions below the normal limit, leading to the inability to fight foreign antigens. Immunosuppression results from the death of immune receptor cells or obstruction of vital intracellular pathways for immune response. Besides, immunosuppression can result from certain diseases or medications that suppress the immune system (Bennett & Gilroy, 2017). Immunosuppressants affect certain inflammatory mediators involved in wound healing. Hypothermia, which can disrupt surgical wound healing processes and increase the risk of wound infection, is more likely in immunocompromised patients (Singh et al., 2017). Equally, reduced blood flow stymies wound healing, presumably preventing cells like platelets and monocytes from entering the site in diabetic patients.

    Conclusion

The patient in this case scenario presents symptoms associated with wound infection, including inflammation, redness, and pain in his leg. The wound infection diagnosis is supported by the patient’s use of garden water to clean the wound cut from a string trimmer. Both the trimmer and the trimmer could be a potential host for pathogens causing the infection. Host factors such as tobacco use, overweight, obesity, and Solute Carrier Family 12 Member 2 genes exacerbate wound infection. Equally, immunosuppression could be a predisposing factor for the patient’s wound infection and exacerbation.

My Clinical Experience during the Covid-19 Pandemic

In December of 2019, the world realized an unknown respiratory disease in Wuhan, China. The disease would later be identified as Severe acute respiratory syndrome coronavirus 2(SARS- COV-2) or Covid-19 as it is mostly known. Medical experts’ fears about the disease would later be confirmed when it quickly spread outside China to affect millions of people. Currently, about 35.1 million people in the world have contracted the disease with approximately 1 million fatalities.

As expected, the disease would affect many of the world’s operations. Health care workers are at the center of those affected by Covid-19. The above statement is supported by the different clinical experiences I have undergone due to the pandemic.

I have learned to work under stressful and tiring conditions due to the pandemic. The high number of coronavirus cases increased the workload at the health care facility. The excess workload emerged from the unprepared nature of medical facilities and the world to tackle the disease. For instance, the hospital realized a shortage in the number of health care workers to take care of the patients.

Thus, the situation meant that we had to take care of many patients with the minimum human labor available. Often, I found myself working during odd hours with little instances of rest. The massive workload with little has thus made me experience a lot of stress.

Likewise, the idea that I may contract the disease or transmit it to other family members worries me a lot. As a health care practitioner, I am in the frontline when it comes to handling coronavirus patients. Despite wearing protective equipment such as the face shield, I still worry that several errors can lead to me contracting the disease and succumb to it or pass it to my loved ones.

Moreover, the coronavirus has changed the way we conduct activities in the health care facilities. We are no longer allowed to hold close social gatherings. Likewise, it is now mandatory for us to wear protective gears within the health care facilities. Additionally, I feel somehow isolated as a healthcare practitioner due to different procedures I am supposed to undergo before meeting my family, for instance, self-quarantining before going home.

    Solution

    Medicaid Funding

Medicaid is a program that provides financial health solutions to an enormous number of individuals who are financially incapacitated to finance their health insurance policies. The program has become more complicated since policymakers often use it to address various policy objectives, leading to structural tensions. Nonetheless, despite frequent alterations and challenges, Medicaid has benefited millions of low-income persons across the United States.

Thus, receiving support from the general public, governments – both the state and the governments, and the academic sector. Essentially, Medicaid Health Insurance Program is, despite its shortcomings and encounters, productive to the United States citizens.

    Challenges in Utilizing Medicaid Funds

Although Medicaid has successfully provided health insurance for needy persons in the U.S., it still has multiple financial management challenges. Some of these challenges include improper payments. Improper payments encompass expenditures done for persons not eligible for the program or services not delivered. Reports from the U.S. Government Accountability Office (2020) indicate that such expenses constituted 9.8% of Medicaid spending in the fiscal year 2018, which is an equivalent of 36.2 billion U.S. Dollars.

In addition to that, the ever-increasing trends in Medicaid’s total spending by eligibility group, too, has become a challenge to its fund’s management. This transformation is a consequence of the aging population, the expansion of state programs where demand for improved oversight programs is made, and increased enrollment expenditures.

    Advantages in Utilizing Medicaid Funds

Proper spending of Medicaid funds has lots of benefits not only to individuals and society but also to the Federal government and state governments at large. This is exhibited in Rudowitz, Hinton, and Antonisse’s (2019) article on Medicaid enrolment and spending growth as of the fiscal year 2019/20. Benefits the authors mention include an increase in a variety of state priorities, in which many areas for spending are expanded.

Through proper utilization of Medicaid funds, beneficiaries are well attended to in large numbers and adequately by offering comprehensive life insurance plans. This way, Medicaid thoroughly performs fundamental obligations of catering for the less fortunate in society.

For me, being a helper will also involves getting to known the person, building trust and nurturing a good interpersonal relationship that fosters healing and wellness.

Lastly, I have had the opportunity to help a recovering drug addict, as she tried to get her life together. Getting to understand the reasons that led to drug and substance use was among the best ways of learning to help and personalizing my assistance (Brekke, 2019). Advising her on how to cope up with feelings of not feeling worthy, mending ties with family and friends and getting back to work were also effective avenues of help her.

    • Full blood count (FTC)
    • Hormone levels test

World Health Organization defines anemia as a condition when\re the RBC or their oxygen carrying capacity is insufficient (Alli et al, 2017). It can be classified depending on the RBC characteristics like size, chromia, and morphology or based on the underlying morphology like bone marrow aplasia.

    A.

    Differential Diagnoses

    • Iron deficiency anemia (confirmed)
      • According to Kabotarizadeh et al (2019) microcytic anemia clinically manifests as the presence of trifling, hypochromic, red blood cells in a peripheral blood smear. This was confirmed after the blood smear indicated that JQ had a low MCV of less than 83micorn. JQ also admits she bleeds heavily during her menses.
    • Megaloblastic bone marrow (refuted)
      • Consists of blood disorders whose defining characteristic is the presence uncharacteristic of the morphological appearance of the developing RBCs in the bone marrow. Its cause is deficiency in Cobalamin or folate (Srikanth, 2016). This was ruled out because the smear after bone marrow aspiration did not show a megaloblastic nuclei larger than normblastic nuclei. The patient was also not having any liver disease.
    • Trauma anemia (refuted)
      • The patient denied having had any accident that count have led to excessive bleeding or a medical operation like surgery that could have led to internal bleeding. Neither does the patient report gastro intestinal issues.
    • Anemia- induced hair loss (confirmed) (androgenetic alopecia)
      • Considering that research has connected iron deficiency to hair loss in women, this is confirmed in the present case. Evidence also indicates that iron deficiency hair loss does not scare the hair follicles. Asz-Sigall et al (2016) also notes dermoscopy is an important complementary method that helps skin specialists to diagnose scalp disorders.
    • Alopecia areata (refuted)
      • Alopecia areata is an autoimmune disorder that manifests through the patient’s immune system attacking the hair follicles (Ramos & Miot, 2015). This was refuted because there are no round smooth patches of baldness in JQ.
